The personal information requested on this form is being collected by The Corporation of the City of Stratford under the authority of the Municipal Act and will be used for the purpose of assisting city staff in making a decision on this project and for administrative purposes. All names, addresses, and comments will be included in material available to the public in accordance with the provisions of the Municipal Freedom of Information and Protection of Privacy Act and certain information may be shared with contractors and sub-contractors during this project.
